Transcript Request Forms
Listed below is a complete list of applications/forms available on the JSCC Website.
Curriculum and Continuing Education Transcripts are $5.00 per copy.
If students complete a paper copy/ PDF Request, they must call the business office at (910) 275-6132 to pay their $5 transcript fee. Students will pay online if they complete their transcript request using an online source.
